Our Commitment to Your Privacy
We are required by law to maintain the privacy of your protected health information (“PHI”), to provide you with this Notice of our legal duties and privacy practices, and to notify affected individuals following a breach of unsecured PHI. We are required to abide by the terms of this Notice while it is in effect.
How We May Use and Disclose Your Health Information
We may use and disclose your PHI for the following purposes without your written authorization:
- Treatment — We may use and disclose your health information to provide, coordinate, or manage your chiropractic care, including consultation with other treating providers.
- Payment — We may use and disclose your health information to bill and collect payment from you, an insurance company, the VA, Freeport-McMoRan (FMI), or another third party for treatment and services provided to you.
- Health Care Operations — We may use and disclose your health information for our internal operations, such as quality assessment, staff training, licensing, and business management activities.
- As Required by Law — We will disclose your health information when required to do so by federal, state, or local law, including reporting as required for public health, health oversight activities, judicial or administrative proceedings, law enforcement purposes, or to avert a serious threat to health or safety.
- Workers’ Compensation — We may disclose health information to the extent necessary to comply with workers’ compensation laws.
Any other uses or disclosures of your PHI, other than those listed above, will only be made with your written authorization. You may revoke that authorization in writing at any time, except to the extent we have already relied upon it.
Your Rights Regarding Your Health Information
You have the right to:
- Request restrictions on certain uses and disclosures of your PHI, although we are not required to agree to all requested restrictions.
- Request confidential communications — ask that we communicate with you by a particular method or at a particular location.
- Inspect and copy your health record, with certain exceptions.
- Request amendment of your health record if you believe information is incorrect or incomplete.
- Receive an accounting of disclosures we have made of your PHI, other than for treatment, payment, healthcare operations, and certain other exceptions.
- Obtain a paper copy of this Notice upon request, even if you have agreed to receive it electronically.
- File a complaint with our office or with the U.S. Department of Health and Human Services if you believe your privacy rights have been violated. We will not retaliate against you for filing a complaint.

- Provide you with this Notice describing our legal duties and privacy practices
- Notify you if a breach occurs that may have compromised the privacy or security of your information

We reserve the right to change the terms of this Notice and to make the new provisions effective for all PHI we maintain. If we change this Notice, we will make the updated Notice available upon request, in our offices, and on our website.
